Fit Notes & Self Certification
If you require a certificate after the first 7 days of self-certification, you will be need to arrange an appointment with your GP.
If you require an extended certificate please contact reception and provide details of the request - they will forward it onto your GP.
Self certification (Form SC2)
You can sign yourself off for the first 7 days. Your employer will have a form or you can can print this one off to claim statutory sick pay.
