Confidentiality

We can assure you that anything in your medical records and anything that you discuss with our staff is treated as confidential. This means that you can tell others about your visit, but we won’t.

The only reasons why we may pass on confidential information without your permission would be:

 

  • to provide other healthcare organisations who are involved in your direct healthcare with the necessary information that they require to be able to effectively and safely assist with your healthcare.

  • or to protect you, or someone else, from very serious harm. We would always try to discuss this with you first.

If you do not wish us to share your data with other NHS services please let us know (although please be advised that this could affect your healthcare). We will never share your data with any organisation outside of the NHS without your permission.

Confidentiality - Consent to share information  with a 3rd party

This form is for patients who wish to request formal consent for Drs and Staff of Bideford Medical Centre to communicate test results, discuss repeat prescriptions and all other medical information  with a nominated person.
