Our Primary Care Network Health & Wellbeing team helps patients to improve their wellbeing, alongside the support given to them by the surgery's clinical staff. They help people with a wide range of practical, social and emotional needs and connect them with organisations, agencies and groups that can make a positive difference to their wellbeing.
